Medical Program graduates: If you require proof of completion of a medical program, please note the following:

Transcripts

Available for graduates of:

• CNA (graduating in 2000 to present)
• LVN
• Respiratory Therapist
• Surgical Technician
• X-Ray program

Education Verification Letters*

Available for graduates of:

• CNA (graduating prior to 2000)
• Dental
• ECG
• EMT
• HHA
• Medical Assistant
• Medical Insurance Billing
• Medical Terminology
• Pharmacy Tech
• Phlebotomy
• Physical Therapy Aide (graduates from 2000 to present )

*There are NO transcripts for these programs.

Please print, complete, sign, and return a Transcript and Education Verification Request Form with payment (cash or money orders only). Transcripts and Verification letters are official and will be in a sealed envelope. Graduate will receive an unofficial copy for their files / records at no additional cost.

Education Verification letters for graduates prior to 2000 requires a search through archived records. Interested graduates must print, complete, sign, and return a request form for an Archived Records Search. Payment must also be in cash or money order and fees are indicated on said form. Verification letters are official and will be in a sealed envelope. Graduate will receive an unofficial copy for their files / records at no additional cost.
